To our current and new Evergreen Pediatric Clinic families: We are pleased to announce we are in updating to an Electronic Medical Record (EMR) system.   During this transition we will request you complete updated intake forms prior to your child’s next Well Child Visit (or new patient visit) to ensure we have the most accurate and current information.  It continues to be our privilege to partner with you to provide exceptional care for your children.
